Insurance & Superbills

A Ray of Hope Therapy & Family Resource Center, PLLC does not currently accept insurance. A superbill is provided so you can seek reimbursement through your out-of-network benefits.

Using your out-of-network benefits

• Call your insurance provider and ask about out-of-network mental health benefits for outpatient therapy.
• Ask whether you have an out-of-network deductible and how much has been met.
• Ask what percentage of the "usual and customary rate" your plan reimburses.
• Confirm whether there's a limit on reimbursable sessions per year.
• Ask whether a superbill is required and what it must include. We provide the superbill; you submit the claim.

Private Pay Policies

Credit card on file. A credit card on file is required for all clients to cover session fees (billed within 48 hours), late cancellations, or no-show fees.

Cancellation policy. Appointments canceled within 24 hours or missed without notice will incur the full session fee. More than two late cancellations or no-shows within a six-month period may result in termination of care, as consistent attendance is essential for ethical and effective treatment.
